What to look for when choosing a microneedling provider in Portland

If you've been researching microneedling in Portland, you've probably gone down the device rabbit hole. SkinPen versus this pen, FDA-cleared versus not, 12 needles versus 16. It's easy to spend hours comparing devices (not to mention the other details like radiofrequency, PRP, exosomes, whatever is the latest and greatest add-on). We argue that although these details are important, the biggest variable that determines the results you will get from your microneedling treatment is who is holding the pen, and what do they know?

We offer medical microneedling in SE Portland at M'illumino Natural Medicine, and we’ve come up with a little guide to help you make your decision, whether you book with us or not.

What can go wrong, and why

If you've spent any time browsing microneedling online, we don’t have to tell you that there is some concerning microneedling treatment happening out there in the world. We consider swollen, bleeding faces, and skin that looks injured rather than treated to be a red flag. We hope that Portland clinics and med spas aren’t sending clients away like this, but we want to inform you that this is not only unnecessary when it comes to microneedling, it is undesired and possibly damaging.

It is not generally the device that is driving this kind of response. Rather, it is a lack of training combined with an attitude that more is more. Aggressive depth does not produce better results. The magic of microneedling is in the body's response—the magical healing process that is prompted by the treatment. This is a concept that makes sense to us with our acupuncture backgrounds, but it can be foreign in a culture where more is almost always assumed to be better.

In our practice, we consider bleeding unnecessary and undesirable except in the deepest work, which is typically reserved for significant acne scarring. Even then, what you should see is minimal pinpoint bleeding, not a dramatic wound response. Redness should resolve largely within 12 to 24 hours, and fully within 48. Swelling should be minimal throughout. If you are seeing photos of someone's face significantly swollen and bleeding after a microneedling treatment, that is not an aggressive treatment working hard. This is a risk of damage and potential scarring, which is precisely what we are not looking for when coming in for skin treatments!

What thoughtful medical microneedling actually looks like

As acupuncturists, we can’t help but be really curious about your body, and your skin is such a cool part of that! When you come in for microneedling, and before the device ever touches your skin, we are reading your skin. We assess texture, pore size, the depth and width of any lines, and the apparent age and thickness of the skin. Thin, crepey skin on the neck, for example, may actually warrant deeper penetration than skin that appears more resilient — which surprises many people. A forehead is not a cheekbone. The area under the eye requires entirely different parameters than an acne scar on the jaw. Stretch marks on the abdomen allow us to go deeper than we would even on a deep acne scar on the face. Treating every zone at the same depth is where providers cause uneven results, prolonged inflammation, and sometimes lasting damage.

Depth calibration is only part of it. Speed settings, number of passes, what is applied to the skin during treatment, and aftercare all shape the outcome as much as the needling itself.

Why licensed acupuncturists are needle experts

Here is something most people don't consider when researching microneedling providers in Portland: what is their actual training with needles?

Licensed acupuncturists in Oregon are regulated by the Oregon Medical Board and hold graduate-level degrees requiring four years of medical acupuncture schooling covering needle technique, tissue response, contraindications, and the body's healing mechanisms at a depth that a weekend microneedling certification course simply does not cover. We have also completed additional training specifically in cosmetic techniques including medical microneedling.

We are, by training, needle experts. Microneedling is a natural extension of that expertise.

We have hundreds of happy patients and a track record we are proud of. We see visible results in almost everyone within one to three sessions. In 14 years of practice offering acupuncture and cosmetic acupuncture, and 6 years offering collagen induction therapy in Portland, we have had zero adverse events or lasting damage from any of our treatments. If you want to know what real results look like, our Google reviews and Instagram are both a good place to start.

What about PRP and exosomes?

You might be seeing microneedling offered with PRP (platelet-rich plasma) or exosomes as add-ons. PRP is a well-established therapy that uses growth factors from your own blood to support the healing response — it has a solid track record and is worth discussing with your provider if you have significant scarring. That being said, we have worked on hundreds of deep acne scars and seen excellent results. We are microneedling minimalists, and thoroughly convinced of its efficacy without the use of PRP.

Exosomes are newer and worth understanding before you pay a premium for them. Injectable exosome therapies are not FDA approved as of 2025, and we'd encourage you to ask hard questions before agreeing to anything injected or implanted.

Topical exosome serums are a different category. We use Plated, a platelet-derived exosome serum sourced from FDA-regulated donation banks, applied at the end of treatment to support the skin's healing response. It's one of the most carefully sourced products in this space, and we chose it because the science behind it is sound and the sourcing is transparent.

As always, we will tell you exactly what we are using and why before anything touches your skin.

Questions to ask before you book microneedling in Portland

Regardless of where you go for microneedling, ask these:

  • What depth range do you work in, and how do you decide? A provider who gives you a single number for every treatment has not thought carefully about this.

  • What kind of results do you typically see, and over how many sessions? Honest providers will give you realistic expectations. We see results in one to three sessions in almost everyone, but we often recommend 4-6 sessions as a full series, depending on a person’s skin and goals. For deep acne scarring, 12 sessions is often warranted. After a series of sessions, folks will often move to a maintenance schedule, coming in once a season, or for a refresh when they feel like they could use it.

  • Have you ever seen adverse events or skin damage from your treatments? Ask directly.

  • What is your training and licensure? There is a significant difference between a licensed medical provider with formal needle training and someone who completed a certification course.

Is microneedling right for you?

Medical microneedling is well suited for fine lines, skin texture, enlarged pores, acne scarring, hyperpigmentation, and general skin quality. We have seen it work wonders for crepey skin on the neck, and also stretch marks.
